Weird, For me it just works :-
/etc/init.d/ipw394d restart
I do see the results you post on occassion - but the following gets
things back on track:
/etc/init.d/net.eth1 stop
/etc/init.d/ipw3945d stop
killall wpa_supplicant
killall ipw3945d
modprobe -r ipw3945
modprobe ipw3945
