After upgrading to Plasma (KDE5) I have a problem with my dual monitor
setup.
When I start/restart a new session both monitors are "displayed" "on
top" of each other (see Screenshot_20160915_201441.png). After
"dragging" DVI-I-1 to the right of DVI-D-0 in SystemSettings (see
Screenshot_20160915_201601.png) everything looks OK.
Any suggestions why this is happening (some file-permission some where?)
and where to search for a solution?

Quick guess, you are running "stable" kde plasma (5.6.5)?
There is a bug there that is fixed in later versions.
I am using 5.7.4 and haven't seen that issue anymore.

Yes, you are correct, I'm running stable (5.6.5).
Then I'll have to wait for next release to "go stable" or try upgrading
to 5.7.4.

I've been running 5.7.4 without any obvious issues for a while now...
Attached is the "package.accept_keywords" file I use for 5.7.4
It's up to you to decide, but every time you log-out, you'll have the same
issue with 5.6...

I upgraded yesterday from 5.7.4 to 5.7.5. I hoped the problem I have had
been fixed, but no. If I switch desktops too many times, or too often or
something, the desktop becomes unusable until I reboot (I haven't tried just
restarting xdm). It flickers rapidly, bits of one desktop are shown in the
middle of another or on a blank background, and it's hard to see just which
is the currently active desktop.
This may be a bad interaction with the xorg-driver for my AMD GPU, which has
only been on the market for about a year. Though sys-firmware/amdgpu-ucode
hasn't changed since June.

I switch around a lot between 4 virtual desktops and 2 displays.
Haven't experienced that myself.
Difference: Using Intel CPU and Nvidia GPU.

I often find files owned by "root" in my $HOME... This could explain
some oddities tho I didn't see your effect yet.
You can quickly find such files and directories with
# find $HOME -user root
I'd also check your /etc/X11/xorg.conf (and .../xorg.conf.d directory),
modern installations usually do not contain screen configuration there.
You may want to clean up there if there's something configured.

It's only needed because I'm booting with EFI GOP and the VBIOS defines
my TV as the primary monitor resulting in very very tiny fonts if I
don't change the order to use "DFP-1" first.
Actually, I want my system to clone both outputs - that is why I
position both on offset +0+0 statically. But upon login, KDE will apply
whatever I defined in kscreen.
So maybe you have some script running which "unapplies" this during
login after KDE has set the settings? Check you login session scripts.

As a workaround, uninstall kscreen, create a shell script (named say
xrandr.sh), and put the appropriate xrandr config in it, mine for
example is:
#! /bin/sh
xrandr --noprimary --output DVI-I-2 --left-of DVI-I-1
Go to the KDE system settings, under startup and shutdown, and add this
script in the autostart section as "Pre-KDE startup".
This will result in your monitors being placed correctly under 5.6.x
until you can upgrade to a version with the kscreen bugfix (at that
time, remove this script and reinstall the kscreen ebuild).
