This looks like fs corruption. Boot a LiveCD if you need to and run fsck.e= xt4=20 =2Dc /dev/sdX. If it comes up with errors you'll probably need to rerun fsck with addition= al=20 options and hope that it fixes the corruption. You may lose some data.
