To try it out once, you can do it like this: ebuild /usr/portage/sys-apps/mkinitrd/mkinitrd-3.5.7-r3.ebuild unpack change things in /var/tmp/portage/sys-apps/mkinitrd-3.5.7-r3 ebuild /usr/portage/sys-apps/mkinitrd/mkinitrd-3.5.7-r3.ebuild merge The cleaner way would be to modify the ebuild and put it into your overlay (along with the modified mkinitrd-3.5.7-dietssp.patch), but on the other hand the bug might already be fixed anyway when you emerge the package the next time.
