Do you have support built-into the kernel for this device:

All these bits should be in there NOT as modules if you're booting off an array that's on one of those controllers.
If you cannot build it into a monolithic kernel.. then you'll have to make sure all this stuff gets into your initrd image.. which I can't help you with. I *ALWAYS* build all disk-access bits directly into the kernel.
