Per-package enabling (package.use) feels a bit safer to me. It's also not a global USE flag and is used with different meaning (e.g. in swaybg, rxvt-unicode, w3m), though that might be more of an argument for default enabling for all packages in gnome profiles, as gdk-pixbuf is there anyways.
