You only masked qt-4.2.1-r1, not qt-4.2.1, so the same error occurs. Put 
oe of these in /etc/portage/package.mask

~x11-libs/qt-4.2.1
>x11-libs/qt-4.2

The first masks all revisions of qt-4.2.1, the second masks everything
greater than 4.2, which is the more brutal approach.
